In this webinar, we will define transition and present issues that can act as barriers to self-management and transition from pediatric to adult care. We will discuss ways to approach the transition of care process, including the importance of active involvement from patients, parents and multidisciplinary staff from an early age.

Biographies

Brittany Savage

Brittany Savage is a nurse practitioner at Indiana Hemophilia and Thrombosis Center (IHTC) in Indianapolis, Indiana. She received her Bachelor’s of Science in nursing at Purdue University in 2009 and her Master’s degree at Purdue University in 2015. Brittany entered hemophilia care in 2011 at IHTC as a clinic nurse and moved into her role as a bleeding disorder nurse practitioner in 2015, specializing in transition from pediatric to adult care. Brittany provides patient care to adolescents and young adults with bleeding disorders and leads IHTC’s transition program. She is a member of the National Hemophilia Program Coordinating Center (NHPCC) Transition Work Group and served as a quality improvement coach in 2016 for the American Thrombosis and Hemostasis Network (ATHN)/Dartmouth Institute Microsystem Academy quality improvement collaborative focusing on transition.
